Transport interventions modify substance fluxes in the oral system to improve transport balance. The main ones are: dental sealants (which occlude the dentinal tubules to reduce permeability in tooth sensitivity and the pits and fissures of the molars to prevent caries), salivary substitutes and salivary stimulants (pilocarpine, cevimeline) to increase flow in xerostomia, controlled drug release systems (chlorhexidine chips, tetracycline fibers) to maintain high local concentrations in the periodontal pocket, and fluoride varnishes (which act as a reservoir for F ions that are slowly released into the enamel). Drug release from a controlled release device follows the kinetics of the Higuchi equation when diffusion is the controlling mechanism. Occlusion of dentinal tubules by sealants reduces permeability proportional to the fraction of occluded tubules. The salivary stimulant acts on the muscarinic receptors of the glandular acini, increasing flow in a dose-dependent manner.
