Biological diagnosis evaluates the state of the oral microbial ecosystem and the host's immune response. Methods include: microbiological culture (S. mutans and Lactobacillus count for caries risk; periodontal pathogens for periodontal risk), real-time PCR for detection and quantification of specific bacterial species, analysis of gingival crevicular fluid (GCF) for markers of inflammation (IL-1, TNF-, PGE, metalloproteinases MMP-8), measurement of leukocyte activity and detection of antibodies. The total bacterial load of the biofilm follows a modified logistic growth kinetics; Sequential analysis of the microbiome (16S rRNA) identifies the dysbiosis profile. The RANKL/OPG ratio in the GCF predicts periodontal destructive activity. Commercial tests (CRT bacteria, Dentocult) allow semi-quantitative estimation of the level of salivary S. mutans and Lactobacillus. Measured variables: CFU/mL of S. mutans and Lactobacillus, [IL-1], [MMP-8], [RANKL]/[OPG]. Determined values: biological risk level, dysbiosis index, severity of inflammation.
